community services
The Brain Injury Rehabilitation Trust (BIRT) also offers community services for people who are at
the final stage in the process of rehabilitation following
acquired brain injury.
BIRT supported
housing offers people the chance to live in the community
but with all the support they need 24 hours a day. Locations of supported housing.
Home support is also widely available across
the country to support people living in their own homes. We
can set up home support virtually anywhere
in response to need.
The goal of community services is to make sure
that progress made during residential rehabilitation is continued
once someone is back at home.
Before someone is offered either supported housing
or home support, BIRT will make an assessment visit.
We also liaise closely with social services and other relevant
agencies to make sure the service offered meets a service user’s
individual needs.
