Service users
The model of clinical assessment and rehabilitation is designed to meet the needs of people who require post-acute rehabilitation, where service users are admitted directly from neurosurgical or acute medical/surgical services, and also of those people seeking community integration rehabilitation.
The aim is to provide a continuous link for each individual through their pathway of care from in-patient services through to re-integration into community. back to top
